To facilitate a smooth process, we kindly request arriving a few minutes early to complete necessary forms such as the medical/dental health questionnaire and patient information form, along with any required financial information forms, if not previously submitted.
You can submit them prior to your appointment here.
It is advisable to take routine medications as prescribed before your appointment and avoid taking discomfort medications 4 to 6 hours prior to your visit.
Please bring the following items for your first visit:
- Your dental insurance card for our records
- Any written referrals or x-rays provided by your dentist
- A list of your current medications
Your initial visit will involve a comprehensive examination and assessment of your case, including a review of information shared by your dentist. Diagnostic x-rays may be taken to thoroughly evaluate your situation, enabling us to tailor an optimal treatment plan for your specific needs. Your doctor will discuss the diagnosis, treatment options, and outline a personalized solution. We encourage you to ask any questions during this discussion.
Our treatments are administered with local anesthesia, and alternative options can be discussed based on your preferences or concerns. If you have any medical conditions such as diabetes, high blood pressure, or specific medical history (e.g., artificial joints, heart valves), please inform us beforehand. For patients requiring antibiotics due to existing conditions, kindly contact our office before your appointment.
For patients under 18 years old, we kindly request their parent or legal guardian to accompany them during their visit.
